PTI Quality Containment Solutions
This page documents the SMS opt-in process used by PTI Quality Containment Solutions (“PTI QCS”) for employees who voluntarily choose to receive employment-related and operational text messages.
PTI QCS obtains SMS consent offline through a physical paper consent form during employee onboarding.
Employees do not opt in to SMS messaging merely by providing or maintaining a mobile phone number in CRIS ERP. A mobile phone number may be stored in CRIS ERP for employee records, but SMS messages are only sent to employees for whom SMS consent has been obtained and recorded.
How Employees Opt In to SMS
PTI QCS uses the following process to obtain affirmative SMS consent:
Step 1 — Employee Receives the Consent Form
During employee onboarding, the employee is provided with a physical consent form containing a dedicated section for SMS communications.
Step 2 — Employee Provides a Mobile Phone Number
The employee may provide the mobile telephone number at which they wish to receive employment-related SMS messages.
Providing a mobile phone number by itself does not constitute consent to receive SMS messages.
Step 3 — Employee Reviews the SMS Disclosure
Before opting in, the employee is presented with the SMS consent disclosure explaining the types of messages PTI QCS may send and the applicable messaging terms.
Step 4 — Employee Affirmatively Selects SMS Consent
The SMS consent checkbox on the form is unchecked by default.
Employees who wish to receive SMS messages must affirmatively check the SMS consent box themselves.
SMS consent is voluntary and is not required as a condition of employment.
Step 5 — Employee Signs and Dates the Form
The employee signs and dates the form to document their selection and acknowledgment.
Step 6 — Consent Is Recorded
After the completed form is received, PTI QCS records the employee’s SMS consent status.
The employee’s mobile number may then be used by CRIS ERP to send authorized employment-related and operational SMS communications.
CRIS ERP is the messaging and employee-management platform used after consent has been obtained. Entering a mobile number into CRIS ERP does not, by itself, constitute SMS consent.
SMS Consent Disclosure
Employees who affirmatively opt in agree to receive SMS text messages from PTI Quality Containment Solutions (PTI QCS) at the mobile number they provide.
Messages may include employment-related and operational communications such as:
Work schedule and shift notifications
Shift changes or scheduling updates
Attendance-related notifications
Assignment or work-location information
Operational updates
Employee notifications and reminders
Other employment-related communications from PTI QCS
Message frequency varies. Message and data rates may apply.
Employees may reply STOP to any SMS message to opt out of future SMS communications.
Employees may reply HELP for assistance.
SMS consent is voluntary and is not a condition of employment.
Employees may decline SMS consent and continue the employee onboarding process.

The form shown above and available through the PDF link is the actual mechanism used by PTI QCS to obtain employee SMS consent.
There is no public online SMS signup form, and employees do not opt in simply by entering a telephone number into CRIS ERP.
Relationship Between SMS Consent and CRIS ERP
CRIS ERP is PTI QCS’s employee-management and operational communications platform.
The roles of the consent form and CRIS ERP are separate:
Physical Employee Consent Form
Used to obtain the employee’s affirmative consent to receive SMS messages.
CRIS ERP
Used to maintain applicable employee information and send authorized operational and employment-related SMS communications after consent has been obtained and recorded.
The presence of an employee’s mobile telephone number in CRIS ERP does not independently establish SMS consent.
PTI QCS maintains records of employee consent so that the company can determine whether an employee has authorized SMS communications.
Opting Out
Employees may withdraw SMS consent at any time.
To stop receiving SMS messages from PTI QCS, employees may reply:
STOP
After opting out, the employee will no longer receive SMS messages from the applicable PTI QCS messaging program unless the employee subsequently provides valid consent again.
Employees may reply:
HELP
for assistance with SMS communications.

Privacy of Mobile Information
PTI Quality Containment Solutions respects the privacy of employee mobile information and SMS consent records.
Mobile phone numbers and SMS consent information will not be shared with third parties or affiliates for marketing or promotional purposes.
Information may be shared with service providers when necessary to provide and operate the SMS messaging service or as otherwise required by law.
SMS consent information is used for the employment-related and operational communications described on this page.

This page is publicly available to document and verify the SMS consent process used by PTI Quality Containment Solutions.
For clarity:
SMS consent is obtained through a physical paper consent form during employee onboarding.
The SMS consent checkbox is unchecked by default.
Employees must take an affirmative action by selecting the SMS consent checkbox.
Providing a mobile phone number alone does not constitute SMS consent.
